      July - September 2015: Fernwood Community Unit, Sherwood Forest Hospital TrustA GP led community inpatient rehabilitation unit with close MDT working including physiotherapist, nursing staff, doctors, therapy assistants and wider hospital and community staff. Experience working with a wide range of physical and mental conditions who where referred from the community (step up) or sub acute setting (step down). The role included access and home visits as part of rehabilitation planning, and complex understanding and liaison with wider referral sources for ongoing community rehab and support for individuals post discharge. Treatment included therapy groups, individuals sessions and equipment provision. September – December 2014: Physical Disabilities Team, Nottinghamshire County CouncilAdult Health and Social Care (18-65 years old) community based working visiting service users within their own homes. Experience working within a large multi-disciplinary physical disabilities team and wider professionals including; health colleagues, housing associations, charities and wider social care professionals. The role involved assessment of functional disability with an emphasis on aids and adaptation provision, with ongoing referrals made to wider social care and mental teams where necessary. May 2014-July 2014: Bladon House School, Newton-SolneySpecialist ASD and associated behavioural difficulties residential school for 5-19yrs. Working within a wide inter-disciplinary team including education, care, social services, physiotherapy and speech and language therapy. Gained experience using a range of assessment tools including Movement ABC and Beery VMI. Designing and implementing individual and group treatment plans with a variety of students with a particular focus on sensory integration and developmental principles. Show less
