      Based on the Stepped Care model of mental health support, Connect to Wellbeing focuses on integration of the health care system and facilitates people moving easily between programs as their support needs change. Funded by the Australian Government through the Primary Health Network (PHN) Northern Queensland Program.- Intake of referrals from GP’s- Refer consumers to low intensity support, psychological therapies, suicide prevention, or refer to Connect to Wellbeing clinical team for assessment.- If psychological therapies does not meet the consumers need then referral is made to more appropriate external supports (for example Partners in Recovery, homelessness services, family violence services).- regular communication with consumers, GP’s and general practice staff, and Connect to Wellbeing clinical team.
