      Amedisys Home Health & Hospice was a Nationally Recognized Home Health and Hospice company primarly focusing on the needs of our Senior populational. My target market was skilled nursing facilities,assistedlivingfaciltiessonal care homes, physician offices and outpatient clinics. My call points were case managers, discharge planners, directors of nursing, directors of wellness, referral coordinators, physicians, nurses, etc.Our service line included skilled nursing, physical therapy, occupational therapy, speech therapy, aides(ADL's) and hospice/palliative care for the homebound patients in the Berks, Schuylkill and Lebanon Counties. Accomplishments/Awards: -Developed a comprehensive Marketing Plan and SWOT analysis to help stimulate revenue growth by 15% of goal for the first full year in business.-Successfully introduced Cardio Com(Tele-Health System) in the Genesis Healthcare Facilities. I successfully launched this in all of there Cardiac Rehab Units to help grow our referrals to over 20 per month just from these facilities.       Links 2 Care/Epic Healthcare is a premiere home health care agency for children with special medical needs. My position as a Regional Home Care Specialist/Liaison was to call on various referral sources, as well as, Hershey Medical Center Children Hospital to increase pediatric revenue. On a daily basis, I would work with discharge planners/case managers, local governmental agencies and physician offices who would refer their families to me. I met with the families to explain how we could be of help. This was always a big decision for the families. After all they were placing the care of their special needs child in the hands of our nursing and professional teams. My approach was to focus on compassion and needs of the child to strive to become a resource for the family.
