  The clinical pharmacist practice perform direct patient care functions with a physician’s supervision.pharmacists have the authority to make changes in therapeutic regimens as necessary. The clinical pharmacist assess a patient’s condition and initiate or modify drug therapy in the areas of pain management, nausea and vomiting, electrolyte replacement, and total parenteral nutrition (TPN) through the use of standardized protocols. The pharmacist is also authorized to prepare drafts of chemotherapy drug orders, clarify existing chemotherapy drug orders, manage other medication-related toxicities or therapeutic concerns, and arrange discharge medications.The pharmacist’s major contributions are selecting the most appropriate pain medication for the patient, adjusting the dosage to optimize pain control and minimize adverse effects, changing intravenous pain medications to oral ones to facilitate patient discharge, When a patient is close to discharge,the pharmacist starts to arrange the patient’s discharge medications. This effort includes calling prescriptions in to the patient’s pharmacy and providing medication education to the patient and caregivers.The clinical pharmacist has the responsibility for promoting maintenance and/or restoration of medical status, designing and modifying some treatments according to needs of the patient. the clinical pharmacist has the responsibility of direct patient care and often function as a member of a multidisciplinary team Show less
