  In my role at the Jackson County Health Department, I served as a liaison or link between health and social services and the community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. I worked with the health department’s Disease Investigation team to connect individuals impacted by COVID-19 to resources, including food, housing, and utility assistance. In order to serve Eastern Jackson County’s exurban and under-resourced areas, my coworker and I implemented a program called connectHERE to create more touch points in the community to increase referral activity and connection to services in Buckner and Grandview. In addition to traditional CHW responsibilities, I helped implement and manage the health department’s referral system, IRIS.
