Responsible for accurate collection of data, analysis, reporting and presentations as well as interdisciplinary communication and education.o Planned and organized the implementation of the CARF restructure and Performance Improvement programo IP Rehab Unit did not have a stroke certification. Implemented a project and they now have a 3 year stroke certification as well awards/certifications for CARF, Joint Commission, CLIA, ACR, CAP.o Created a Risk assessment tool that mimics the Joint Commission, was approved by the Quality Safety Council and is now being utilized in all of the East Region of St. Luke’s.o Implemented a process to survey patients at time of discharge and 60-90 day post discharge for IRU. A 50% return rate on the surveys has been achieved to date, higher then counterparts of other St Luke areas.o Co-chair of the pain team. Created an audit tool for progress around pain and are now at 90% compliance.
