      My role involves conducting a face to face assessment of patients experiencing mental health crisis, which includes monitoring and taking vital Physical observations such as Blood pressure monitoring, Temperature, heart rate and blood glucose monitoring. Working with Psychologists, Psychiatrists, behavioural Therapists and clinicians to create a robust risk assessment and plan of care for patients accessing the Service.Working with Doctors and Pharmacy to manage the delivery of psychotropic medications within the community and monitoring the effect of these medications on patients mood, mental state and sleep pattern.After recovery, sign-posting patients to relevant services to meet their psychological and emotional well being or to whatever follow-up the service user may require.
