  Bilingual Intensive Services Case Manager

  An Intensive Case Manager helps clients access the required financial services, residential services, psychiatric services, social services, medical services, and other crucial services. As a CM, I focus on all client outcome reports, client registration forms, service document forms, and more. As a CM, I complete all client service plans while focusing on their requirements and strengths. I also monitor client service delivery by remaining in contact with service providers and clients. Provide clients support with basic ADL's (shopping, payee services, budgets, managing and attending appointments). Coordinate services while teaching life skills promoting goal attainment, crisis management, recovery, rehabilitation and resiliency to underprivilege populations. Encourage and counsel clients to follow through with drug rehabilitation and continuous psychiatric therapy.
