  I successfully manage care for adults with a wide variety of diagnoses including post­operative orthopedic surgeries, cardiac telemetry monitoring and various medical diseases/conditions. Everyday I develop individual care plans including assessments, interventions and nursing diagnoses for each of my patients depending on their needs. I believe educating patients and families is an important aspect of nursing and I focus my education on nutrition, pain management, prevention of nosocomial conditions and discharge planning. I enjoy working closely with interdisciplinary teams, such as, case management, physical and occupational therapy and physicians to effectively manage holistic patient care. Establishing a trusting relationship with my patients is always an important goal that I strive to achieve each day. I utilize the educational opportunities offered through my hospital to improve my knowledge of nursing care, quality improvement and evidence ­based practices. Show less
