  Utilized strategic thinking, problem solving and collaboration with Interdisciplinary Team, assess and evaluate the quality-of-care residents are receiving and provide reports to federal/state per regulations. Utilized critical thinking along with extensive clinical knowledge and experience to implement and update plan of care/care plans for incoming and current residents. Successfully led an 8-person team, including 1 LPN, 3 nursing assistants, 1 Restorative Aide, 1 Recreational Assistant, 1 Dietary Assistant, and 1 Environmental Staff, while providing direct care for as many as 30 residents. Interacted directly with physicians to ensure all care orders were explicitly followed. Coordinated transfer of residents from our facility to the hospital as necessary. Performed ongoing assessments of residents’ physical and mental health. Explained procedures/treatments to residents. Documented resident falls, infections, skin concerns, as well all changes in status, according to facility policy.
