Show less -Serves as a consultant for the advanced heart failure team-Based on standards of care provided by Joint Commission, coordinates a multidisciplinary team to insure delivery of adequate and fiscal accountable care.-Coordinates inpatient and outpatient VAD evaluations, provides patient/family teaching, staff education as pertinent to the VAD evaluation process-Provides inpatient management for the VAD/advanced heart failure population by completing daily progress note, assessing patient, and assisting the medical team formulate patient treatment plan.-Coordinates research activity for the program-Implements process improvement processes based on quality results, specific to VAD specific infections, neurologic events, emergency care, palliative care involvement, and equipment management-Developed process for rapid destination therapy VAD evaluations, which provided: increased implant volume by 100% in six months, decreased post-implant length of stay, decreased direct cost, increased contribution margin and improved patient outcomes.-Functions as project manager for the VAD program, coordinating various clinical and fiscal initiatives at the discretion of the medical director and service line administrator.-Serves as a consultant for other VAD programs regarding performance/quality improvement.
