  • Worked one on one with individuals with traumatic brain injuries (ratio was 1:8); built relationships with participants, legal guardians and caregivers in order to develop person centered plans that addressed the participant’s short term goals. • Tracked 13 program participants progress on a monthly basis and provided quarterly reports.• Created a personal goal tracking system for participants allowing them to monitor themselves, increasing engagement in activities and showcasing the positive impact of the program overall. • Identified community resources and coordinated with outside agencies to provide additional support to program participants; incorporated opportunities for participants to engage in volunteer activities.• Collaborated with participants to develop and enhance administrative assignments that built self-confidence and added value to the Clubhouse program.
