      I am also called for in case of short staffing to work on the floor either as a nurse manager or a medication pass nurse. My job aside from accurately administering the patients' medications and treatments is first and foremost to assess their needs and health problems using the nursing processes. I develop, review and update the plans of care for each and every patient on my shift. Monitor their response and progress vis-a-vis the care plans that was centered and explained to them. These comprised their physical, mental, emotional and social well being as well as evaluating their vital signs, laboratory and diagnostics results ordered by the physician. Show less

      Case Manager/ Nurse Manager

      Case Manager is an important part of the healthcare system that encompasses from an admission of a patient from the acute care setting like the hospitals to a sub-acute or skilled nursing facility for transitional care. My job is to continue the goals of care and to improve the quality of life of the patient until the time that they are safe to be discharged back to their homes or to the community. I drive the interdisciplinary team and care providers including the physicians (with my full respect to them!) to achieve those goals and plans of care, keeping in mind that the patient is the center of the team. I also serve as an advocate of the patient and their family members, part of my job is the utilization of resources for durable medical equipment needed, community/government resources, home health/caregiver and reimbursement of payment to the patient's insurance.
