  *Psychiatric Case Management and Discharge Planning on the 60 bed locked inpatient psychiatric unit. I worked with a team of 6 – 8 Case Managers under the supervision of a Licensed Clinical Social Worker*Coordinated placement, aftercare, and transportation for patients leaving the hospital. I collaborated with family, aftercare providers, and/or residential placement staff to assure smooth transition of care. *Addressed questions and concerns of patients and facilitated patient groups on the unit. Was responsible for completing admission paperwork, psychosocial assessments, and discharge paperwork.*Participated in interdisciplinary treatment team meetings including psychiatrists, Medical Doctors, nurses, Recreation Therapists, and pharmacy staff. Supervised medical students on psychiatric rotation, and instructed them on how to complete psychosocial assessments and work within the framework of the diagnostic criteria for mental illnesses.*Supported a staff to open an off-site satellite unit of the hospital including quality control of Policies and Procedures, facility safety, and clinical documentation which resulted in receiving accreditation by The Joint Commission (TJC).   *Provided psychosocial rehabilitation services to people with chronic and persistent major mental illness in their homes and in the community. As lead Case Manager for approximately 7-10 clients at any one time, I was responsible for coordination of care with the other staff members as well as coordination with other members of the client’s interdisciplinary treatment team (psychiatrist, Medical Doctor, lawyer, family, trust officer).*Partnered with a colleague to open the Orange county division of the Life Adjustment Team. We were successful in getting the satellite office operational and fiscally successful within a couple months. This included marketing and building relationships in the new region, which resulted in enough clients to staff six employees within the first nine months of opening. *I interviewed, hired, trained, and supervised a staff of four master’s level trained Psychosocial Rehabilitation Specialists.
