      -Senior Social Worker within the Bexley Integrated Rapid Response Team which is a multidisciplinary Team that responds urgent health and social care needs of Bexley patients within primary and secondary care by providing a rapid holistic assessment of needs within the patients’ home and at emergency departments and acute medical units at the Queen Elizabeth Hospital and the Darent Valley Hospital. Also, assisting the Team Manager in providing a comprehensive integrated service, safeguarding vulnerable adults and promoting rapid support and care for adults and their carers during a crisis. -Provide specialist social work practice skills, and knowledge of adult social care including safeguarding vulnerable adults and promoting care management. -Undertake highly complex care arrangements for assessment and support planning in situation which involve risk, complexity and safety of the service user and their carers. -Work in partnership with service users and their carers, ensuring the provision of a service, including self-directed support, which meets their assessed needs; work in partnership with other agencies in assessing, planning and implementing person centred care to promote choice and independence. -Assist as part of the multidisciplinary team in improving service users’ quality of life and prevent hospital admissions. -To assist and direct team members in assessing the needs of individuals, developing reablement plans, planning individualized budgets and monitoring their progress. -Responsible for ensuring the workload of the service is prioritized appropriately and is allocated to social workers and social care assistants of the integrated team. -Contribute to the development of adult social care provision and policy and ensure effective use of time and resources. -Promote the team to other departments, statutory and voluntary organizations to ensure that referrals to the service are appropriate. Working proactively with the Multidisciplinary Team and Brokerage/Commissioning Team towards the provision of support to adult Bexley residents to facilitate timely discharges from hospital – reablement and conventional packages of care, emergency respite, placement, etc.-Assess patients social, emotional, environmental, financial, and support needs in hospital settings according to the care Act 2014 (holistic assessments). -Complete mental capacity assessments in accordance with the Mental Capacity Act 2005 in order to empower and protect individuals who may lack capacity to make decisions about their care and treatment.-Follow-up reviews at client’s homes under the Discharge To Assess Pathway in order to ensure that clients’ level of care is adequate and ensure their safety in the community. -Assist, as part of the multi-disciplinary team in improving the quality of life of clients in order to maximize their independence and assist with accessing services and resources to provide practical help.-Responsible for making enquiries when a safeguarding concern is raised in order to promote well-being and prevent abuse and neglect from happening; ensure the safety and wellbeing of clients who have been subject to abuse or neglect; and put measures in place in order to ensure their safety.-Complete 4 week placement reviews in order to ensure that the clients’ care needs are being met adequately in the right environment and with the appropriate support. -Supervise Social Care Assistants in order to provide support with their cases, identify learning and training needs and any difficulties that they might be experiencing and establish an action plan to ensure that their goals and objectives are being achieved.
