The team provides case management services and assists them in addressing barriers to access to community-based care. The team offers and provides consultation with licensed clinicians for members who could benefit from their assessment and interventions. The team educates members regarding their disease processes, treatment plans, plan benefits, and community resources. The team positively reinforces healthy life-style changes and engages members in all appropriate services for their biopsychosocial needs, using Motivational Interviewing techniques. As the Director of this team, I lead a local, continuous quality improvement process. I am responsible for the daily evaluation of the team’s performance against quality indicators and performance guarantees. I am responsible for identifying root causes for any shortcomings, identifying local and national best practices, educating staff, and implementing improvement plans. I am a member of a national team of like Directors who collectively drive quality improvement activities across the national WPC process.I am a Motivational Interviewing Trainer and I am the Optum point person and subject matter expert for all operational aspects of the national, UHG, maternity case management program.
