  I work at the intersection of long term care, quality measurement, and electronic health information exchange as an internist/geriatrician at Massachusetts General Hospital where I provide clinical care, supervise trainees and conduct research on improving transitions of care and the exchange of clinical information at transitions. At the national level I co-chaired the Long Term and Post Acute Care (LTPAC) work group and was a Lead on the Longitudinal Coordination of Care (LCC) work group within the ONC Standards and Interoperability Framework which developed the data set which now serves as the national standard for transitions of care and the exchange of longitudinal care plans. Currently I am a Lead on the Electronic Long Term Services and Supports work group (eLTSS) which will develop standards for the exchange of an LTSS care plan, a member of the federal Health Information Technology Policy Committee (HITPC) Advance Care Models and Meaningful Use work group, of the ONC Care Plan Task Force within the Office of Care Transformation and a technical advisor to the CMS HIT Standards Committee. I am a member of the recently created HL7 work group to develop a CDA Implementation Guide for Personal Emergency, Critical, and Advance Care Plan Document which will enable individuals to share critical information with the health care system.
