  The Peer Navigator supports members who are experiencing/ are at risk of experiencing homeless with managing their own health in the Long Beach, CA area. Assist in motivating behavioral changes in patients to improve health outcomes through education, peer support, housing & resource navigation, and the relaying of shared experiences.

  Service liasion

  Responsible for ensuring that the participants placed into permanent housing by the Abode Alameda County Housing teams are connected to and receiving services from their service provider. When necessary, the Service Liaison will step in to address pressing housing stability issues and manage housing crises if the service provider is not immediately available.
