At Vandenberg AFB I was the sole rehabilitation and injury prevention subject matter expert for 14K beneficiaries. In my first year as the medical facility's Health Promotions director, I achieved the #1 business plan execution rate among physical therapy (PT) clinics in our region, and was #6 of 63 PT clinics Air Force wide. During this time I forged a direct-access clinic that accelerated access to care for patients with musculoskeletal injuries by 2 weeks and sped recovery by an average of 16 days, significantly alleviating the orthopedic load on our family practice clinic by 47 patients per month. In the direct-access role I operate as a physician extender, am responsible for start-to-finish patient care, and am credentialed and privileged in diagnostic imaging referrals, prescription of musculoskeletal medications, the ordering of lab studies, and sub-specialty clinic referrals. In addition, I undertook a novel data analysis endeavor which established standardization and transparency of musculoskeletal patient care, averting $24K in avoidable and redundant costs across 32 civilian PT clinics who support our DoD patient beneficiaries (see bell curve below). Furthermore, our Health Promotions team developed a unique survey of nutrition supplement users that was immediately requested by the Air Force Medical Operations Agency as well as the Army, Navy, and Marines, establishing a DoD-wide benchmark which is facilitating healthy, informed dietary supplement choices for service members and their families. Finally, our Health Promotions team again leaned forward with an FMS/SFMA-based injury prevention initiative that identified 56 at-risk service members. Systematic injury risk interventions have salvaged an estimated 700 lost duty hours due to injury and rehabilitation. This effort earned recognition by the regional Command Surgeon General who requested we produce a formal investigation toward a potential service-wide impact. Good times! Show less In April of 2013, I was selected for the position of Squadron Vice Commander. The position was a supporting role to the Squadron Commander in addition to my role as Surgical Services Flight Commander, and developed from my successes as Operations Officer and Emergency Operations Center Team Chief. During this time I had extended opportunities to lead and oversee all planning and procedures of the Medical Operations Squadron, including preparing 4 different medical clinics for a highly successful medical facility re-accreditation inspection, and a top rating of "highly effective" by an Air Force inspection team. Our work to standardize organizational procedures and increased efforts toward patient-centric process improvement led to our being recognition as the Biomedical Team of the Year. Some of my most rewarding experiences during this time came as opportunities to directly support individuals in need of assistance or guidance, as well as formally recognizing personal and team successes. Show less While stationed in Oklahoma, I was team chief for the medical facility’s Emergency Operations Center (EOC) during an EF-5 Tornado. This position increased both leadership and department manager duties. The EOC acts as the central command-and-control hub during low-frequency, high-risk threats, whether they be natural disasters, man-made, etc. The local destruction from the tornado resulted in a “Major Disaster” declaration by the POTUS. During this event the EOC team was responsible for critical disaster management which integrated local and federal organizations, including support for coordinating search operations for missing personnel, manpower and equipment allocations, and salvage assistance for displaced families. As a result of these efforts, the Humanitarian Service Medal was awarded to the men and women of Tinker Air Force Base in March of 2015. "Established in January 1975, the Humanitarian Service Medal is presented to honor members of the armed forces who distinguished themselves by meritorious direct participation in a significant military act or operation of a humanitarian nature or have rendered a service to mankind." Show less In addition to my position as Surgical Services Flight Commander, I was entrusted with increased administrative responsibilities as Operations Officer for the squadron. This assignment evolved out of my clinic-specific successes with targeted metrics and data analysis. The expanded role enabled me to interact closely with Group Practice Managers, as well as the Chief of Medicine and Chief Nurse, resulting in medical team process enhancements and standardization that immediately increased efficiency and productivity. As a direct result of my data analysis and subsequent recommendations, all family medicine and pediatric clinic teams implemented administrative and procedural changes that increased productivity 19% and generated and additional $301,000 in just 3 months. Show less During my time in Oklahoma City I directed a team of 24 personnel in Physical Therapy, Orthopedic Surgery, General Surgery, & Chiropractic Services clinics. This team included 2 orthopedic surgeons, 2 general surgeons, 4 physical therapists, and a chiropractor. When I arrived, the team consisted of a general surgeon, chiropractor, and 4 PTs. The organization I arrived at had decided the previous year to expand its services to create the most broad spectrum of medical care possible in order to meet the needs of a large and diverse patient population. Having hired 3 additional surgeons and support staff, our new "Surgical Services" clinic developed a level of teamwork and synergy I have experienced on few occasions. -I established all surgical patient intake and discharge procedures de novo as well as with the help of similar-sized surgical clinics already in place-I initiated and implemented "resource sharing" relationships with 2 civilian hospitals in Oklahoma City that allowed our 4 surgeons to perform inpatient and outpatient surgeries for military personnel. These relationships optimized the workload capacity and created additional revenue for the supporting civilian hospitals. This provided substantial taxpayer savings by utilizing civilian medical resources and personnel already in place rather than building similar facilities from scratch. In our first fully operational year as Surgical Services Clinic our team cared for 13,000 patients. In addition, we developed a wellness model focusing on preventive cancer services, increasing life-saving operations 71% in one year. Show less The Human Performance Flight Commander serves the 28th Medical Group of Ellsworth Air Force Base, an ambulatory healthcare system providing for 16,000 beneficiaries, As Flight Commander I directed Physical Therapy, Medical Nutrition Therapy (MNT), and the Health Promotions wellness clinics. Primary duties include oversight of all musculoskeletal rehabilitation, smoking cessation, nutrition, wellness, anger and stress management programs. Our wellness programs produced $527,000 annually and 18% of all medical clinic encounters for a facility that generates $5M in healthcare. The process improvement programs I implemented resulted in a 92% increase in MNT use, a 6% increase in productivity despite losing 33% of PT staff, and a Health Services Inspection rating of "Excellent" for all wellness programs, directly contributing to the Command Surgeon General's award of Best Medical Clinic in 2010. As the lead instructor for AF Physical Training Leaders base-wide,my fitness programs produced a 89% passing rate in just 30 days for AF members who had failed. These programs were a continuation of the comprehensive program I developed that was implemented across all AF Europe bases. These efforts resulted in the award of Physical Therapist of the Year for the Air Combat Command. As a clinician specializing in manual therapy, I became the first physical therapist in the Air Force to become certified and credentialed in Trigger Point Dry Needling. As a result, I became the lead consultant for standardization of Dry Needling to the director of AF physical therapy and the AF Medical Operations Agency. This work increased the scope of practice for all 140 AF PTs. Likewise, my Nebraska licensure afforded me the opportunity to act as a consultant to the Nebraska Physical Therapy State Board, which formally recognized Dry Needling as being within the scope of physical therapy practice for all therapists licensed in Nebraska, increasing the scope of practice for 1600 PTs. Show less In my second year as a licensed therapist I became solely responsible for the pre and post-operative evaluation, design, and execution of the hospital's cardiac rehabilitation program for high-risk cardiac patients at Wilford Hall Medical Center, a level 1 trauma center. Our team was just one part of a multi-disciplinary process that included nutritional counseling, lipid and diabetes management, smoking cessation, stress management, and mental health. As the hospital's only privileged Advanced Cardiac Lilfe Support therapist, I was personally responsible for the restructuring and redesign of this program that ultimately led to a 49% decrease in post-operative rehabilitation time, significantly reducing the patient's hospital stay as well as associated medical costs. In addition, I conducted BLS training and cardiopulmonary education and training for 14 therapists and 21 support staff. Show less
