      -Serving as an advocate for the complex, comprehensive care needs of PACE-enrolled frail seniors at a nursing home level of care, who enroll with the wish of remaining safely in their own communities for as long as possible.-Performing comprehensive nursing assessments in the adult day health center's (ADHC) clinic upon enrollment, every 6 months, or with change in participant condition (including post-fall, hospitalization, or discharge from a facility).-Serving as the panel registered nurse to 65 participants on an interdisciplinary team (IDT). -Care planning for participant needs per CMS-regulated PACE guidelines, and presenting at bi-weekly IDT meetings to discuss care issues and attain care plan consensus. -Triaging participants with urgent or emergent care needs in the ADHC or via telehealth, utilizing critical thinking to escalate to provider or other disciplines as needed.-Following up, and reporting to the IDT on overnight events, including ER visits and hospitalizations, as reported by the on-call provider. -Providing participant and family education on wound, foot, and skin care; edema, blood pressure, and glucose management; medication adherence; and chronic disease management.-Utilizing professional in-person and telephonic interpreter services to ensure understanding, as appropriate.-Placing internal referrals for specialist appointments, e.g. dental, audiology, podiatry, and optometry per participant needs.-Providing pre-instructions for external appointments and alerting providers of any necessary medication holds. -Endorsing participant care needs to, and collaborating with, the home health team to deliver the most appropriate care to participants. -Collaborating with staff in skilled nursing facilities (SNF), residential care facilities for the elderly (RCFE), memory care, and assisted living facilities when such a level of care has been deemed appropriate.-Weekly comfort care/end-of-life (EOL) check-ins as appropriate.
