      The role of Care Manager is to be the communication link between our patients and the health care team. A primary function is to advocate for the patients and families and address any issues or concerns they may have. It is important to remove any barriers they feel they are experiencing in order to achieve self-management of their chronic diseases. It is also important to assist the patients as they transition to ensure that these various moves will be seamless

      Internal Medicine Transitional Care Manager

      This position works with our patients who have been discharged from our institution and have an external primary care physicians (PCP). A follow-up phone call is completed after discharge to the patients and their PCP office to ensure continuity of care. If any issues or needs are identified, they are addressed. The transitional care manager remains available for ~ one week to provide ongoing assistance as needed.
