This also often involves collaboration with Multidisciplinary Team Members, as well as Child Protective Services and Law Enforcement.- Addresses barriers to care by linking patients with appropriate resources for health insurance/prescription assistance programs, transportation, mental health/substance use treatment, housing/utility assistance, employment, and food. This may also include addressing safety/neglect concerns.- Conducts full psychosocial assessments with individuals and families through use of a culturally sensitive, strengths-based approach that is developed in partnership with the individual/family. This also includes conducting safety and risks assessments to improve overall wellness and care. - Engages in brief counseling with families who are experiencing grief and loss, or have difficulty coping with a new diagnosis. Also engages in motivational interviewing and problem-solving methods to support improved treatment adherence. - Has strong documentation skills and is very knowledgeable about HIPAA laws and the 21st Century Cures Act.
