In this role, occupational therapists provide assessment and relevant interventions to improve or maintain a patient’s optimal functional capacity and assist in discharge planning.• Assessed patient using SCGH Initial Assessment and completed further assessments in performance of functional transfers and ADLs, cognition (including functional cognitive assessments and MoCA), and falls risks as required.• Consulted family members for collateral when appropriate to ensure a complete picture of the patient was gathered to aid in assessment and intervention.• Applied clinical reasoning to formulate a plan based on the information gathered, and made recommendations for discharge planning, including interventions and education to support discharge home/rehabilitation/placement.• Liaised with the multidisciplinary team to share information and advocated for patients to safeguard their needs. Support Community Care Mangers and Home Care Package Coordinators with complex clinical clients prior to discharge from hospitals to promote a safe and effective discharge. Contributed to quality improvement projects such as updating documentation to ensure compliance with changes to funding streams. Liaised with external stakeholders to ensure access to subsidised equipment and home modifications in line with the Commonwealth Home Support Guidelines. This resulted in one provider changing their processes to support informed consent.
