      Family Nurse Practitioner, Primary Care: Providing primary care to all ages at a Federally Qualified Health Center (FQHC). Carrying an independent patient panel, ~20 patients scheduled daily, for acute, chronic, and preventative care. Providing a full continuum of primary health care services to a medically and socioeconomically diverse population.• Led a provider group and wrote the policy for including gender-affirming care (GAC) for transgender non-binary persons, including hormone treatment, into our primary care services at Generations.• Leader in the provision of TeleHealth primary care services during the COVID-19 pandemic and beyond. • Hepatitis C Treatment Program: 2016-Current, Lead provider of Hepatitis C evaluation, management, and treatment at Generations’ Willimantic clinic, working in collaboration with ID specialist. Currently developing standardized provider protocol for Hepatitis C testing and treatment and post-treatment primary care recommendations for Generations. • Patient Engagement Liaison, Patient Transformation Network: 2017 -2019 Provider and patient liaison for Generations’ Willimantic clinic site, actively engaged in design and implementation of grant initiatives for data-driven process and quality improvement, as part of the Patient Transformation Network grant, which was awarded to FQHCs in Connecticut through the Community Health Center Association of Connecticut (CHCACT). This grant was part of the larger Transforming Clinical Practice Initiative (TCPI) launched by The Centers for Medicare & Medicaid Services (CMS). • Designed and implemented population risk-assessment tool and process for risk-stratification and provision of care coordination services. • Collaborated on review, improvement and implementation of current clinical screening tools (childhood development, substance abuse, domestic violence) to improve patient care and data collection for quality measures.
