As a Care manager I was responsible for creating care plans that reflect the needs of various families. I was the liaison between the families, community providers, and Dependency Court. I worked alongside the Care Coordinator to insure the families had whatever services and supports were available in the community while simultaneously providing reports to the court to ensure the safety of the children and progress of the parents remained the focal point.
