  The interviewing and assessment of individuals with mental health and/or substance use disorders as it relates to homelessness and navigates them from homelessness into permanent supportive housing programs. Provided supportive services in the form of housing assessment and readiness counseling, community case management, substance use treatment referrals and counseling, community integration skills,community resources coordination. Maintained accurate HMIS data as one of the administrators for my agency as well as fill in for important meetings for my Supervisors when they were not able to attend. Participate in Workshops, Conferences, and training regarding MSHDA Housing Choice Voucher programs and other community-based housing programs. Facilitate groups regarding housing and ending homelessness. Responsible for the input and maintenance of data in the Michigan Homeless Information System (HMIS). Assist PSH Coordinator in administrated and daily coordination of duties for the Permanent Supportive Housing Team. Show less
