  Program Coordinator

  As a Program Coordinator under the guidance of the Program Manager, I collaborate with frontline care providers and care coordination staff to support individuals living with HIV (PLWH). My role focuses on identifying newly diagnosed individuals, those at risk of falling out of care, and those who have disengaged from care, successfully navigating them back into treatment. I assess and screen clients for barriers to care engagement, including social determinants of health, unmet substance use and mental health needs, and chronic medical conditions. I conduct client intakes into supportive programs, such as Health Home, and provide motivational interviewing, adherence support, and behavioral counseling as needed.
