Stressed Loco Pilots may take wrong judgments and that may leads to fatal accidents in Indian Railways. Recent Balasore Triple Train Accident in Bihar is the crystal clear evidence of Human Error ( Human Failure) in Indian Railways and the Prima Facie is Workmanship Failure leads to fatal accident and leads to loss of hunders of passengers lives.
