Providing direct and indirect support to staff within the TAP program through client advocacy, consultation and support with complex multi-system files/ transitions (including support to barriered high risk young adults), and assisting with the creation, development, and implementation of effective policy and practice relating to service delivery to young adults accessing TAP moving between provinces or territories. Working collaboratively with the partner (south) TAP Transition Specialist to ensure best practice and consistent service delivery is offered to young adults currently accessing services through (or seeking to access the services of) the Transition to Adulthood Program across the province. Identifying trends related to multi-system service delivery gap areas in young adults transitioning in or out of the TAP program, and working collaboratively with partner agencies to rectify and resolve these areas. Relaying real-time feedback from frontline staff and young adults accessing TAP to TAP's Policy and Practice Specialists to adjust or update policies to continue to enhance service delivery. Delivering presentations to TAP staff and community stakeholders across the province in order to build collaborative working relationships.
