   Manages all aspects of the business including client care, care planning, support to clients and their families. Communicates and advocates for clients as they interact with medical practitioners. Assists clients to verbalize their medical issues.  Plans for safe hospital discharges with social workers and other discharge planners to be sure that appropriate care at home is available, assists in finding the best possible rehabilitation centers. Advocates for best possible patient care in assisted living facilities, nursing homes, rehab centers and hospitals. Assists with finding appropriate options for clients moving to assisted living or skilled nursing facilities. Finds caregivers, physicians, therapists and other medical practitioners as well as practitioners of alternative and complementary therapies who are best suited to work with each individual client. Provides community education to elders and their families. Advocates for people with diseases causing dementia to be sure they are safe, cared for, respected, and treated with dignity at all times. Makes referrals for all aspects of elder life: everything from financial planners and specialist in legal issues concerning seniors to finding dog walkers and movers.
