  I served as a Case Manager for youth adjudicated delinquent. For non-custodial youths who are placed on probation, I monitored their compliance with the court's requirements of probation. In addition, I monitored school behavior and performance, behavior at home and in the community, and completion of community service work or other conditions of probation. For youth placed into DCS custody as delinquent, I served as the local case manager. Responsibilities included: family engagement, assessment of the youth and family members, treatment planning, facilitation of home visits for youths, and documentation of all activities relevant to the youth's case in the state's SACWIS (Statewide Automated Child Welfare Information System). Other duties:-travel to facilities for face to face meetings with youth, provider staff, and at times with family members -occasional transportation of family member(s) to see the child)-regular contact with the youths' schools, alcohol and drug treatment providers, mental health providers, and family members. -court appearance in juvenile court regarding a youth's charges received while on probation, compliance or lack thereof, and or content and appropriate permanency plan goals and action steps Show less
