      -Train program staff at head office or in the field on monitoring and evaluation, how to use standardized data-collection and reporting tools, data-cleaning, and data-entry.-Maintain the M & E tools for the integrated service delivery program (ISDP) for the project and provide hard copies of the tools to the clinical supervisors or health facilities that need them.-Lead the weekly IDSR and monthly HMIS data collection and submit to the county health department and the state ministry of health.-Carry out data quality checks (DQA) based on the developed schedule.-Assist the County Health Department in monthly data entry into the District Health Information System (DHIS) software, validate and provide feedback regarding the quality of the data.-Present data in tabular form to the project manager, County health department and other technical staff, for inclusion in narrative reporting.-File hard copies of monthly data from health facilities as well as checklist used for supervision.-Ensure that copies of reports to the CHD, SMOH are prepared and sent and covering letter signed.-Perform spot-checks and on-site-data-verification (O-S-D-V) at the facility level and then data submitted for data quality.-Submit monthly data to the IMC M & E team in the head office.-Collect, analyze and document progress and outputs of monthly and quarterly review of target vs. achievements-Together with the program manager, write the quarterly narrative report.
