  Care Manager

  Responsible for developing and coordinating the care program at DPCA; I assess client’s (benefactor) and family’s unmet health and social needs, and then develop a care plan based on mutually agreed goals with clients, family representative, and health providers including emergency plan, medical summary, and on-going action plan as appropriate. Ensures client's confidentiality and quality of care is maintained. Supervises and monitors client’s engagement with their care plan and progress towards the expected goal in a timely fashion, and facilitate changes as needed. Directly works with the volunteers on the outcomes on the field and its progress; schedules and delegates caregivers to any of our clients. Recruits, trains and supervises volunteering caregivers for care assignments. Serves as a point of contact, advocates, and provides informational resources for client, family, care team and community. Liaise and maintain partnerships with primary healthcare centers, faith-based organizations and local community to ensure quality and best practices. For safety and quality, I recognize and respond to opportunities for improvement and professional development. Make sure to participate in care coordinator related trainings and meeting activities for continuous profession development. Ensure the team complies with the professional standards and health safety regulatory requirements. Show less
