  Covering two GP practices in Clifton area, three days a week as well as the prescribing hub adjacent to the practice:- Medication reviews for patients with long-term conditions and multi-morbidity (SMRs- polypharmacy and de prescribing)-Targeted medication reviews: hypertension (diagnose and management), lipids management (primary and secondary prevention),contraception, HRT, gout, MSK pain (acute of diagnosed origin and chronic), mental health (depression/anxiety, sleeping disorders,ADHD), dermatology (acne, eczema, nail fungal infections), minor acute conditions (herpes simplex) previously diagnosed whichrequire re treatment-Spevial interest in T2DM, reversal through diet and GLP1 agonists- Reviewing blood tests for patient with diagnosed chronic conditions as hypertension as per local protocol- Prescribing in accordance with the BNSSG formulary and NICE guidance- Prescribing duty (Monday mornings for The Family Practice)- Resolve prescription queries for patients, colleagues and community pharmacies- Medicines reconciliation, review and prescription issue following patient discharge/clinical letters- Liaising with NHS/private healthcare providers (eg ADHD 360, Gender GP) if clarification needed regarding clinical letters/SCPs- Drug safety monitoring using tools as Radar Eclipse and the Safety Dashboard- Actioning MHRA alerts and legislation updates for the practices- Managing QOF indicators (asthma, AF, HF, lipids)- PAIs monthly submission (based on FP34 and appendix item searches)- Work closely with ICB pharmacist in order to complete NHS BNSSG ICB projects and advise colleagues on appropriate and costeffective prescribing (eg Bisphosphonates project 2020; Lipid project 23/24; Diabetes project 23/24, Hypertension review 24/25)- Mentoring pharmacy technician on CPPE pathway (now completed)- Supporting new starting clinical pharmacists and pharmacy technicians with training and feedback-Covid-19 vaccination clinics Show less
