      I was the Team Coordinator for the St. Paul Program. This is a residential site, that is partnered with the Canadian Mental Health Association (Niagara). Our purpose focuses on transitioning our participants back into the community. We work with individuals that have an ABI, and also have a mental health diagnosis. My role specifically focuses on placement screening, case management, program admission and discharge, staff supervision, scheduling, and participant support. As the Rural Services Coordinator, it was my job to connect participants who have been diagnosed with an Acquired Brain Injury, with community resources that can help them regain their independence. I was also responsible for case management, life and social skills training, reporting, and completing assessments.
