Monthly disease state monitoring and counseling to employees identified as having diabetes or pre-diabetes. Taught 6 group classes per year in addition to monthly monitoring. Provided physical assessments and recent test review at each individual visit. Adjusted drug therapy as needed. Utilized Cholestech LDX and GDX machines to perform point of care testing. •Obtained CLIA waived status in 2002 allowing point of care (POC) testing of waived tests at the pharmacy for cholesterol, A1C, blood glucose, liver function monitoring. •Pharmacist run immunization program: initiated and implemented pilot program in 2002. Program accepted by corporate starting 2003. Personally trained first two years of volunteer pharmacists. Mandatory pharmacist participation started 2014. •Implemented free blood glucose meter program in partnership with Lifescan.
