  In this position I am be split between two different locations taking referral patients in the areas of...- Prediabetes/Diabetes- Hypertension- Family Planning / STI prevention- Tobacco Cessation programming- Weight Management- Basic Nutritional and Physical EducationAs well as patient health education I am in charge of running multiple programs within my two locations.1) Food pantry project: - Using grant funded money to combat food insecurity in my locations by setting up a food pantry. Data was then tracked anonymously and used for studies.2) National Diabetes Prevention Program: - Recruited specific populations under eligible criteria and facilitated year long, in-person lifestyle development sessions to help combat diabetes in participants through healthy lifestyle choices.3) Family Planning Only Program: - Recruited eligible uninsured patients to the Apple Health - Family Planning Only Program. Program supplied participants with fully covered FDA approved contraceptives', STI testing, cervical cancer screenings, and annual women's wellness exams.Was on the board for Clinical Quality Measures in my clinic.- Successful completed clinical quality measure for Colorectal Cancer Screenings giving patient information/education on FIT KIT testing.- Successfully completed clinical quality measure for Hypertension Control <140/90 through patient education and vitals tracking.
