Review intake, applications, and other participant forms for completeness and accuracy. Input case notes.● Attend meetings to review any pertinent information, such as new policies/statistics/procedures issued by the city or related organizations.● Travel throughout the community, meeting with housed clients in their homes or with homeless clients in various places including shelters.● Transport clients to drug testing for employment and/or to fill out employment on-boarding paperwork, pick up medications, and grocery shopping, etc.● Connect clients to resources, make referrals, and assist as necessary - applications for housing, employment, or other programs.● Advocate on behalf of clients, such as by appearing in court on behalf of a client who may have violated probation in order to explain how the client is working to get their life back on track. Write letters to court, lawyers, or other case managers requesting permission for client to attend retreats, field trips, or life skills workshops.● Handle administrative tasks, such as the maintenance of case notes and any other related duties as assigned by the Executive and/or Program Directors.● IDOC GTL registration and visitation scheduling. May have to speak with facility scheduler for various reasons.● Ultimately, must be flexible enough to accommodate rapidly shifting priorities. For instance, might expect to meet with a client about a housing voucher or job fair, only to discover that the client hasn’t had a meal in a long time.
