      Clinical Care Coordinator (CCC) conducts a mental health assessment, mental status exams, and a person-centered strength assessment to determine medical necessity and diagnosis. The CCC works with social service agencies, community partners, naturally occurring resources, Peer Support, and stakeholders to ensure that individual recovery goals are met.Essential Job Functions:Conduct Mental Health Assessments and Person-Centered Strengths Assessment and develop an individualized person-centered treatment plan with the individual and natural supports that is focused on recovery goals including community integration.Provide personal disclosure statement after Supervisor approval and explanation of service options, risks and benefits to each client to obtain informed consent and document that this has been done in a progress note.Delivers therapeutic services using evidence based/promising or best practice approaches with individuals, families, natural supports, and groups as determined by the individual in the treatment plan. Able to demonstrate fidelity and outcome measures using agency provided tools and measures.Practices using Hope Inducing Behaviors.Coordinate services and communication with community providers, natural supports, and key stakeholders as appropriate. Facilitate obtaining PCP, other medical assistance, housing, employment, socialization, money management, therapy, and fiscal benefits etc. using naturally occurring resources.Provide crisis prevention and intervention services which may include responsibility for rotation on 24/7 support phone.
