I was given one huge plastic bag as a product sample to be analyzed. My team checked every product that given to us and analyzed with Fishbone Method and Pareto Chart. After finished, we give advice and feedback to the company for decrease the defect rate of their product.

HIRARC document will be used to give solution for the company to minimize the work accident.PT. SPINDO II is a company which manufactures steel pipes and located at Rungkut Industri Surabaya. I work at PT. SPINDO II as a internship student at HSE Department. My internship project is to prepare HIRARC (Hazard Identification Risk Assessment and Risk Control) document. HIRARC is one of the requirements to applying OHSAS at the company.
