      In my current position I work with a range of various healthcare professionals in a multidisciplinary team. Part of my role includes conducting screening of patients to determine whether to get heavily involved from the point of admit to hospital or whether to continue to monitor until significant social issues arise. For patient’s that require more intense social work support I conduct comprehensive psychosocial assessments. These assessments include, but are not limited to, probing deep in to individual’s living circumstances and functional abilities by asking curious and thoughtful questions with an anti-oppressive practice. A large part of my position involves communicating with and referring patient’s and families to a range of community service providers based on their individual needs. Dealing with complicated health issues and family matters in a non-judgemental and professional manner is something I pride myself in while performing my job. At the heart of this position is ensuring that I actively plan safe and appropriate discharges of patient’s to alternative levels of care in the healthcare system and elsewhere in the community.
