  I worked on an intensive case management team that served to assist in stabilizing individuals with severe and persistent mental illnesses that were most at risk of ongoing crises and inpatient hospitalizations. The ultimate goal was to ensure successful integration and ongoing stabilization in the community. This was achieved through bi-weekly/weekly one-on-one interactions, securing stable housing, promoting recovery from mental illness through medication management (weekly medication delivery and monitoring), providing transportation needed to promote continuity of care, connecting services, and advocating for consumer welfare and rights. As a CTT case manager, I also served as a liaison between different agencies such as the criminal justice system and the Social Security Administration and the consumers with whom I worked.   -Conducted 1:1 monthly supervision with 4 full time coordinators; quarterly and as needed for 25+ resident counselors. -Facilitated resident counselors' hiring and training which consisted of 2 days worth of vigorous training and shadowing. -Developed a 3 hour training course mandatory for direct care staff in efforts to continually improve quality and person-centerness. -Became certified as a Certified Psychiatric Rehabilitation Practitioner to help facilitate best practices within the supported houses. I was responsible for directly supervising 25+ part-time resident counselors and four full-time Psychiatric Rehabilitation Coordinators. These individuals provide direct care to individuals with mental illnesses who reside in supported housing. The four full-time Psychiatric Rehabilitation Coordinators provide psychosocial rehabilitation within the homes to facilitate the recovery journey. The psychosocial rehabilitation includes one-on-one training as well as the facilitation of several groups a day focused on various topics that are instrumental in ensuring successful transition into independent living. Show less As the Supervised Residential Services Coordinator, I was responsible for managing a referral list and scheduling and performing intakes into our supported housing facilities. Prior to admission to two of our supported housing facilities, pre-certifications were written and submitted to insurance entities for authorization for specified level of care and continued reviews were submitted for continued placement as needed. During this time I was also trained to facilitate IMR curriculum (Illness, Management, and Recovery) in the houses. Show less
