      Case manager

I provided case management services to at-risk and homeless youth ages 16-21, by helping them apply for DHS assistance, obtaining documentation (social security card, birth certificate, state ID), and guiding them into AIY housing program or looking for housing elsewhere.

I was on one of two fire teams, which means that my team and I completed two rounds (out of four in total) of prescribed/ controlled burns. I am red card certified as a wild land firefighter. My team has completed prescribed burning in South Carolina, participated in disaster relief work in Tupelo, MS, worked alongside a trail crew on the Cumberland Trail in TN to complete an access trail, completed many beautification tasks in the city of Rolling Fork, MS, provided service for Habitat for Humanity in Eustis, FL, and worked in Okefenokee Swamp in Southern Georgia maintaining waterway trails and working alongside the fire crew. Program assistant/ facilitator

Step by Step is a non-profit that is an Outreach Program of the VOA. Step by Step's mission is to empower women who are, have been, or are at risk of being incarcerated to claim their strengths and build healthy lives for themselves and their families. Step by Step facilitates groups in the Albion and Henrietta Correctional Facilities, and we also hold a STRIDE meeting for women on "the outside" and provide strength-based support to those who attend. Currently, Step by Step is also researching ways in which the programs may be useful for men. Part of my role is to oversee this, translate workshops from those which are women-focused into something that resonates with men, and facilitate two groups a week for a small group of men who are chronically homeless, yet in a permanent housing program. I also perform Outreach regulalrly. I attend meetings in the community, as well as meet with folks one-on-one in order to build connections and learn more in depth about programs that are good resources for the folks in our program. Health homes care manager

This program works specifically with adults, ages 18 and over, who are have Medicaid insurance and who also have qualifying health conditions and at least one need. As a care manager, I have a 50 person case load and help to coordinate the care of these individuals to help them achieve social, physical, and behaviorial health goals, while in the long run, lowering emergency room visits and costs for the state.Health Homes is a non-intensive approach, but very much person centered and we work with folks to help empower them to create goals for themselves and achieve them. Big Brothers Big Sisters helps children achieve success in school, helps them avoid risky behaviors such as getting into fights and trying drugs and alcohol, and helps them improve their self-confidence. We hold ourselves accountable to our supporters by regularly measuring our impact. As the Brevard County Manager, I focus my efforts in Brevard County enrolling both youth and volunteers into the program, conducting orientations for the families and volunteers, matching youth with a volunteer, and following up with matches in order to provide match support to ensure child safety and to mentor those in our program. I am constantly networking to work on future events that may take place in Brevard County, as well as to further recruit potential volunteers and families for the program. Exibir menos
