      • Conducting meetings with service providers, consumers and guardians to review and renew consumer’s individualized service plans on a quarterly or annual basis. • Conducting service monitoring each month in the consumers’ home (ISL or group home) to ensure that consumers are receiving the services authorized by the Department of Mental Health through the Medicaid waiver program. This includes: reviewing daily documentation notes, inspection of the home and reviewing medical documentation. • Conducting quarterly reviews of monthly documentation provided from service/residential providers including: monthly progress summaries and nursing reviews to ensure that the consumer’s medical needs are being assessed monthly by a registered nurse and that documentation is present that shows whether the consumer is making progress towards outcomes as defined in their individualized service plan. • Writing individual service plans about the consumer, following guidelines and directives as outlined by the Department of Mental Health. • Documenting episodes of care on each consumer daily in CIMOR (Consumer Information Management, Outcomes, and Reporting) and billing to Medicaid. • Creating yearly budgets for consumers that receive residential or other paid services and submitting them to be authorized to the state regional office. • Meeting consumers and their families and assessing the consumers needs. • Conducting assessments on consumers as needed or required by DMH • Writing plans and creating budgets requesting services through the Medicaid Waiver Program and submitting them for the Utilization Review at the state regional office. • Administrative duties including: faxing items, sending and receiving e-mails, taking and placing phone calls, mailing out documents, preparing items for filing in the consumers records, maintaining consumer working files, attending trainings, traveling, and maintaining caseload tracking information.
