  The Vulnerable Populations Program Manager provides leadership, planning, implementation, and organization to oversee the Vulnerable Populations Program (VPP) initiatives that are specifically designed to provide health care solutions that overcome the social and systemic forces that either drive VPs away from care or limit their ability to access care. The Medicine Service Line (MSL) defines Vulnerable Populations as persons or communities that are susceptible to reduced health, housing, financial or social stability outcomes because of current experience of or historical exposure to trauma, violence, poverty, isolation, bias, racism, stigma, discrimination, disability or chronic illness. This definition focuses on how systems distribute benefits and burdens in ways that create or perpetuate disparities within our community. The VPP Manager liaises with health system and community partners and develops and oversees the development of programs that connect the community’s vulnerable with health care services to manage chronic illness.Some priority areas:• Maintaining and evolving relationships with key community-based committees and boards that directly impact the effectiveness of the role • Development of social determinants of health (SDoH) infrastructure that drives referrals, services, and transparency throughout the health care journey – meeting patients where they’re at in life.• Development of a robust funding stream built in large part through grant programs and foundations• Creation of dependable Business Intelligence data that captures and reports on Diversity, Equity, Inclusion initiatives• Partnering with the Diabetes Care Pathway Team to incorporate innovative and sustainable access for VPs • Discovering the best approach for long-lasting partnership with Lancaster’s Hispanic Community• Development of short term, crisis housing for homeless patients discharged from the emergency department needing continued or follow-up care Show less
