  Assisted 18-24 clients daily in all aspects of their daily living. Responsibilities included but were not limited to representative payee, budgeting, food and clothing shopping, psychiatric follow-up appointments, housing, SSI-SSD applications, SSD hearings, Medicaid, Rental Assistance applications, medication monitoring, employment opportunities, primary care doctor appointments, specialist appointments and court appearances. Main objectives were to keep my client list functioning in society with minimal to no psychiatric hospital re-admissions. Requirements were to be familiar with each individual’s baseline behavior and recognize any deviation in that behavior and act accordingly. Sometimes this meant coordinating with the police department to come and get the patient and transport to the psychiatric screening center at the hospital. Follow-up visitation in the hospital was necessary if the client met placement criteria. Other duties were to conduct individual assessments of possible new clients meeting psychiatric case management criteria at the psychiatric wing at Atlantic Regional Medical Center and Ancora Psychiatric Hospital. I have knowledge of the DSM IV along with psychiatric medications. Documented all interactions with each client by written progress notes and weekly team meetings. Attended numerous psychiatric workshops on BiPolar Disorder, Schizophrenia, and Borderline Personality Disorder as well as self defense training. Show less

  I represented three nursing and rehabilitation centers in Cape May County as the initial point of contact between prospective patients and their families and our facility admissions. Role included sales and consultant responsibility for Ocean View Rehabilitation Center, Eastern Shore Nursing Home, Courthouse Convalescent Center, Residence at Ocean View and Magnolia Adult Day Care. These facilities served the short and long term rehabilitation needs of five secondary feeder hospital systems. I was required to maintain a thorough working knowledge of the variety of services offered at each facility which included but were not limited to: Sub Acute Rehab, Long Term Care, Specialized Alzheimer/Dementia Needs Adult Medical Day Needs, Assisted Living Needs, Drug and Alcohol Program Specialties Specialty Care (ie: Peritoneal Dialysis, Hemodialysis, Tracheostomy Care, TPN, Peg Tubes, and Wound Vacs) I effectively promoted all services to the necessary referral sources and update any changes as needed. I confidentially coordinated medical data from the hospital to the facility of choice, completing a professional and in depth assessment of the patients needs by utilizing standard and customized Pre Admission forms. I ensured all state required forms and financial documentation was provided from the hospital to the referring facility (i.e.,; PAS, PASRR, insurance authorization numbers and contacts for managed care contracts, updated MAR, discharge summary, and carve out agreement letters). I presented financial options to the patient or family members and coordinated care, which sometimes involved special medical equipment to the facility as needed. I maintained exceptional relationships with hospital physicians, discharge planners, social workers, and families. I adjusted communication style to fit varied situations, cultures, referrals, and residents while demonstrating compassion and preserving dignity, privacy and confidentiality. Show less
